Enrollment

If you are interested in enrolling your student(s) in QDA for the 2011-2012 school year, please follow these steps:

  1. Fill Out the Enrollment Packet.

    Please email info@qdaenrollment.org to request an enrollment packet. Or fill out an application online by clicking here. You will need to complete this packet for your student, including all of the necessary forms (which are listed on the last page) and submit it to one of our offices.

    You must have Adobe Reader installed before you can begin.

    Required forms to be completed:
    Student Application
    Emergency Medical Card
    United States Department of Education Race and Ethnicity Form

  2. Submit Your Application.

    Via Email

    Send each form as an attachment in a single email to applications@go2qda.org. You will receive an automatic notfication email from our system once we receive your email. If you do not receive a notification, please contact our office to ensure we received your application.

    Via Fax


    Fax all of the required documentation to 330-364-0680. Please also include a cover letter.

  3. Congratulations!


    Once we receive your application we will gather your records from your previous school and begin the enrollment process. This process typically take us a couple days, but it depends on how quickly we receive your records. Please call our office if you have any questions about the enrollment process or email information@go2qda.org
